Athletics

Required paperwork: In order for a student to participate in sports at Emmett Middle School, the following items are required. All forms can be printed from the links below or picked up in the EMS office. Please bring the completed forms to the office.

• Sports physical. Students must be cleared for sports activities by a medical practitioner. Only one sports physical is required during middle school. Find the form here. Information on FREE sports physicals here.
• Athletic handbook forms. The forms on pages 6-8 of the athletic handbook (release/consent, concussion acknowledgment, and emergency contact information) once submitted are good for the school year. The emergency contact information should be updated more often if there are changes to phone numbers, insurance, etc. Find the handbook here.
• Participation fee: $35 per athlete, per sport. Payment options include online via the Infinite Campus Parent Portal and in the EMS office via cash, credit card, or check. A $2.50 fee will apply to all credit card payments. Note: For tryout sports, the fee will be added to your student's account after he or she has been placed on a sports team. Non-tryout sports can be found in the Optional Payments section.
